reduced longevity.1 Heart failure can be due to systolic or diastolic left ventricular dysfunction. Most patients with heart failure have primarily systolic dysfunction. In two-thirds of patients with systolic dysfunction, the cause is coronary artery disease.2 However, patients may also have nonischaemic causes of cardiomyopathy such as hypertension, valvular heart disease, myocarditis, systemic disease, toxins, alcohol/drug abuse, or idiopathic cardiomyopathy. Depressed left ventricular ejection fraction (<40%) is the hallmark of systolic dysfunction. Diastolic dysfunction is defined as normal or preserved rest systolic function in the presence of

Controversies in the use of beta blockers in heart failure.

Recent evidence from randomized controlled trials has provided compelling evidence to support the use of beta blockers in most patients with heart failure due to systolic dysfunction. There is little disagreement about the mortality benefit provided by adding beta blockers to standard therapy, which may include angiotensin-converting enzyme inhibitors, diuretics, and sometimes digoxin. A few ar...

Beta-blockers for heart failure: an evidence based review answering practical therapeutic questions.

Beta-blockers are underutilised in heart failure because clinicians may be unsure whether all beta-blockers are useful, how therapy should be initiated and whether beta-blockers are contraindicated in some patients. Bisoprolol, carvedilol and metoprolol succinate have been clearly proven to reduce mortality and hospitalisation in patients with Class II to IV heart failure; limited evidence also...

Poor tolerance of beta-blockers by elderly patients with heart failure

Despite the well-understood importance of beta-blocker therapy in heart failure, it is sometimes not possible to use beta-blockers in elderly patients due to poor tolerance. In this report, we describe the case of an 83-year-old patient with severe systolic heart failure complicated by aortic valve stenosis and atrial fibrillation. A simple therapeutic approach involving discontinuation of beta...

The Use of Metoprolol CR/XL in the Treatment of Patients with Diabetes and Chronic Heart Failure

About 5 million Americans suffer from heart failure. Given the correlation of heart failure with age and the rising life expectancy, the prevalence of heart failure continues to increase in the general population. Sympathetic stimulation intensifies with progressive heart failure. The rationale to use beta-blockers in individuals with impaired myocardial function is based on experimental eviden...

Beta-blockers in chronic heart failure.

Chronic heart failure (CHF) constitutes a significant clinical issue due to its increasing prevalence in general population and still fatal prognosis. During last twenty years the significance of beta-blockers in the treatment of CHF has changed. It resulted from the hypothesis from 1975 that in the course of CHF it comes to the activation of sympathetic system and renin-angiotensin-aldosterone...

Comparative effectiveness of different beta-adrenergic antagonists on mortality among adults with heart failure in clinical practice.

BACKGROUND Randomized trials have demonstrated the efficacy of selected beta-blockers in systolic heart failure, but the comparative effectiveness of different beta-blockers in practice is poorly understood. METHODS We compared mortality associated with different beta-blockers following hospitalization for heart failure between 2001 and 2003. Longitudinal exposure to beta-blockers was ascerta...
